Implementation of Comprehensive Geriatric Evaluation for the Care of Patients with Lymphoma

Abstract

Introduction: Elderly people with cancer must be evaluated differently to define the degree of vulnerability and fragility when facing treatment.

Objective: To implement comprehensive geriatric assessment in patients diagnosed with lymphoma.

Method: A prospective and longitudinal analytical descriptive study was carried out with geriatric patients with diagnosis of lymphoma, enrolled in the Institute of Oncology and Radiobiology, from May 2019 to July 2020. These subjects had a vulnerability index greater than three and underwent a comprehensive geriatric evaluation to define their degree of frailty and risk of toxicity to chemotherapy.

Results: A total of 45 patients were evaluated. The median age was 70 years. 60% had a diagnosis of diffuse large B cell lymphoma. 84.4% of the patients had normal nutritional status. Within the assessment of cognitive status, recent memory loss was the most frequently found disorder. After the comprehensive geriatric evaluation, it was determined that 80% of the elderly were in the pre-frail category according to the age-related chemotherapy risk scale. The low-risk group presented the lowest percentage of patients with grade 3 - 4 toxicity (16.7%).

Conclusions: The comprehensive geriatric evaluation made it possible to detect alterations in the functional, nutritional and cognitive status of the evaluated patients, and to stratify them into risk groups for chemotherapy toxicities and accurately identify the risk of chemotherapy toxicity.
